Permittees who are presently covered under an issued NPDES or OPDES general permit for storm water discharges associated with construction activity may submit a Inspection Request (IR) form when their facilities are getting ready to file a Notice of Termination (NOT). For a construction site, when the site has been finally stabilized (i.e., a uniform perennial vegetative cover with a density of at least 70% of the native background cover has been established for all unpaved areas and areas not covered by permanent structures or where equivalent permanent stabilization measures such as riprap or gabions have been used), and all storm water discharges from construction activities that are authorized by general permit (OKR10) are eliminated, or they are no longer the operator of the facility, an NOT must be submitted that is signed in accordance with Part 4.5 of the general permit. If you submit this IR form to DEQ prior to termination of your current permit, the DEQ will conduct an inspection and provide any assistance necessary within 30 days of receipt of this form. Upon completing the inspection, the DEQ will notify you of any needed changes to the site conditions or that the site has met the final stabilization requirements under the permit. This Inspection Request form should not be substituted for an NOT. You must continue to meet the conditions and terms of the permit until you have filed the NOT. For a Corporation: by a responsible officer, which

means: (i) president, secretary, treasurer, or vice president of the corporation in charge of a principal business function; or their designee, or any other person who performs similar policy or decision-making functions for the corporation, or (ii) the manager of one or more manufacturing, production, or operating facilities, provided, the manager is authorized to make management decisions which govern the operation of the regulated facility including having the explicit or implicit duty of making major capital investment recommendations, and initiating and directing other comprehensive measures to assure long term environmental compliance with environmental laws and regulations; the manager can ensure that the necessary systems are established or actions taken to gather complete and accurate information for permit application requirements; and where authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures.
