The Louisiana Request Form is a document used to request transcripts from Southeastern Louisiana University. It allows students to specify how they want their transcripts delivered, whether by pickup, email, fax, or mail.
The request form is valid for 30 days from the date of submission. If the transcript is not picked up or delivered within this timeframe, the request will be void, and a new form will need to be completed.
To complete the form, you must provide:
-
Your signature (required for the release of information)
-
The date you are submitting the request
-
Your email address or fax number (if applicable)
-
The name and address of where the transcript should be sent
-
The name of a designated person if someone else will pick up the transcript
Can I request more than one transcript at a time?
You can request a maximum of three copies per request. If you need more than three, you will need to submit additional requests.
What happens if I do not pick up my transcript in time?
If you do not pick up your transcript within 30 days, it will be shredded. A new request will be necessary if you still need the transcript after that period.
How will my transcript be marked if I pick it up?
All transcripts that are mailed to students or picked up by students will be marked "Issued by Student." This designation is important for any institutions that may require an official transcript.
You can mail the completed request form to Southeastern's Office of Records at SLU 10752, Hammond, LA 70402. Alternatively, you can fax it to Southeastern’s Office of Records.
Is identification required for picking up the transcript?
Yes, identification is required if someone other than the student is designated to pick up the transcript. This ensures the security and proper handling of the student's records.
